With college, a full-time job and an upcoming wedding to plan, Brooke Vitense had the hectic life of an average young woman in her twenties. But a chance discovery one morning would completely upend her normal life.
That morning Brook noticed white spots on the underside of her tongue while brushing her teeth. Not long after, she pointed out the spots to her dentist during her regular dental checkup. He recommended having the spots biopsied, just to be safe. She needed a wisdom tooth removed, so she scheduled the biopsy with her oral surgeon to coincide with the tooth extraction.
She soon forgot about the biopsy — until her dentist contacted her about the results. The lesions were pre-cancerous: he recommended she have them and a portion of her tongue removed surgically as soon as possible.
She underwent the procedure, but that wasn't the end of her ordeal. The follow-up pathology report indicated cancerous cells in the tissue excised during the procedure. To ensure elimination of any remaining cancerous cells they would need to remove more of her tongue as well as the lymph nodes from her neck.
Brooke survived her cancer experience and has since resumed her life. Her story, though, highlights some important facts about oral cancer.
Oral cancer is life-threatening. Although cases of oral cancer are rarer than other types of malignancies, the survival rate is low (50%). This is because lesions or other abnormalities are often dismissed as simple sores. Like any cancer, the earlier it's detected and treated, the better the chances for survival.
Anyone of any age can develop oral cancer. While most cases occur in older adults, young and otherwise healthy people like Brooke are not immune. It's important for everyone to make healthy lifestyle choices (good oral hygiene and nutrition, moderate alcohol use and avoidance of tobacco) and see a dentist whenever you see an abnormal sore or spot in your mouth.
Regular dental checkups are crucial for early detection. Had Brooke not seen her dentist soon after discovering the spots on her tongue, her survivability could have been drastically lower. Regular dental visits (and cancer screenings if you're at high risk) could mean all the difference in the world.

Oral cancer is a serious health problem, responsible for the death of about one person every hour, every day in the United States. It was once thought that folks over 40 were chiefly at risk for the disease. If present trends continue, however, younger people may soon form the majority of oral cancer patients. So, no matter who you are, it makes sense to recognize the risk factors, and find out what you can do to reduce your chances of getting the disease.
As in many other diseases, genetic factors play a role in determining whether an individual will develop oral cancer. At present, there's nothing we can do about these inborn traits. But there are several choices we can make that will lessen our risk of oral cancer. Most of these risky behaviors are associated with other types of cancer as well.
Moderate to heavy drinkers, and users of tobacco products of all types, are as much as 9 times more likely to develop the disease than non-users. Chronic exposure to the sun has long been associated with the development of cancers of the lip. And, because the sexually-transmitted Human Papilloma Virus (HPV) can lead to oral cancer, unsafe sexual behavior is a factor that's fast becoming a primary cause of the disease.
So if you need another reason to quit smoking, stop drinking excessively, wear sunscreen and practice safe sex — consider this your warning. But there's still more you can do to reduce your risk for oral cancer, and improve your general health as well.
Eating a plant-based, whole food diet doesn't just reduce your risk of getting oral cancer — it also makes you less likely to develop many other cancers, and various chronic conditions like heart disease. The exact mechanisms by which this happens aren't completely understood, but its effects have been documented in numerous studies.
Avoiding certain chemicals, like the nitrites often found in preserved foods, can reduce cancer risk. And the antioxidants you get by eating a balanced diet rich in fruits and vegetables can help protect your body from cancer-causing substances.
Finally, don't ignore regular cancer screenings. The early signs of oral cancer are difficult for many people to distinguish from common mouth sores — but we are trained to identify possible problem areas, and can schedule further tests if needed. Chewing tobacco, especially among young athletes, is considered fashionable — the “cool” thing to do. Many erroneously think it’s a safe alternative to smoke tobacco — it is, in fact, the source of numerous health problems that could ultimately lead to disfigurement or even death.
Chewing or dipping tobacco is especially linked with the sport of baseball. Its traditions in baseball go back to the late Nineteenth Century when players chewed to keep their mouths moist on dusty fields. The habit hit its greatest stride after the surgeon general’s warning on cigarettes in the late 1950s. Now, players wishing to emulate their major league heroes are prone to take up chewing tobacco at an early age.
But the habit comes with a price tag. Individuals who chew tobacco are more susceptible to oral problems like bad breath, mouth dryness, or tooth decay and gum disease. Users also increase their risk for sexual dysfunction, cardiopulmonary disease (including heart attack and stroke) and, most notably, oral cancer.
Derived from the same plant, chewing and smoke tobacco share a common trait — they both contain the highly addictive drug nicotine. Either type of user becomes addictive to the nicotine in the tobacco; and like smoking, a chewing habit can be very difficult to stop.
Fortunately, many of the same treatments and techniques for quitting smoking can also be useful to break a chewing habit. Nicotine replacement treatments like Zyban or Chantix have been shown effective with tobacco chewing habits. Substituting the activity with gum chewing (non-nicotine, and with the sweetener Xylitol), or even an herbal dip can also be helpful.
Like other difficult processes, it’s best not to try to quit on your own. You should begin your efforts to quit with a consultation with your doctor or dentist — they will be able to prescribe cessation medications and provide other suggestions for quitting. You may also find it helpful to visit a behavioral health counselor or attend a tobacco cessation support group.
Rather than just one approach, successful quitting usually works best with a combination of techniques or treatments, and perhaps a little trial and error. The important thing is not to give up: the improvements to your dental health — and life — are worth it.

Chewing tobacco has a certain cachet among its users, especially young boys and men, who believe using it makes them appear macho or “cool.” They also believe this “smokeless” variety (as it's often marketed by tobacco companies) is safer than cigarettes or cigars.
Unfortunately, nothing could be further from the truth. In reality, chewing tobacco is harmful to your health — and especially your oral health. Regular use of these products can lead to severe dental and mouth conditions resulting in disease, disfigurement, or even death.
Like the smoked variety, chewing tobacco infuses its users with nicotine, a chemical stimulant naturally produced by the tobacco plant. The body responds to the stimulant's effect and begins to crave it, leading to addiction.
The problem, though, is the other ingredients in chewing tobacco: more than thirty other substances known to cause various kinds of cancer, including oral. Oral cancer alone is extremely dangerous: many patients suffer partial or complete loss of oral tissue and facial structures, including the tongue, lower jaw or even the face. Some even lose their lives — statistics show that only half of those with oral cancer survive more than five years after diagnosis.
Although cancer may be the most harmful effect of chewing tobacco, it isn't the only one. Researchers have found tobacco users have higher rates of tooth decay and gum disease than non-users. Tobacco also causes cosmetic and hygiene problems, including tooth staining and chronic bad breath.
If you're a tobacco user in any form, and especially chewing or spit tobacco, as your dentist we would advise you to consider quitting the habit. Giving up tobacco will not only improve your oral health and appearance, it may even save your life.

No one wants to hear the word “cancer.” But thanks to advances in detection and treatment, the disease increasingly can be stopped in its earliest stages when it's most treatable and outcomes are most favorable. Oral cancer accounts for a relatively small 3% of cancers in men and 2% in women, but early detection rates are lagging. Our office screens for oral cancer as part of your regular checkups. Knowing some of the signs and symptoms can help you monitor as well.
The main areas where oral carcinomas (cancers) occur are:
- the tongue (most common location, particularly the sides and on the floor of the mouth),
- the lip (especially the lower one),
- the oral cavity (the mouth), and
- the pharynx (back of mouth and throat).
Most oral cancers are preceded by surface changes (lesions) of the oral membranes. In the “precancerous” stage, white or red patches start forming and a non-healing ulcer may appear. The most common locations on the tongue for this to occur are on the sides and underneath on the floor of the mouth. Lip cancers typically develop on the lower lip, usually in people with a history of sun exposure. There has often been prior damage at the site such as scaling and crusting.
Be aware that oral cancers can be mistaken for cold and canker sores, ulcers, minor infections, and even irritations caused by biting or certain types of food. If lesions don't heal within two or three weeks, there's a higher likelihood that they are cancerous.
An in-office examination includes the following:
- visual inspection of face, lips, neck and mouth;
- inspection of sides and underneath of tongue and floor of mouth using gauze to gently manipulate the tongue;
- palpation of the floor of the mouth, sides of neck and glands to detect unusual lumps; and
- an “open wide and say ‘Aah’” examination of the back of your throat.
There are some risk factors for oral cancer that can't be controlled, such as a family history, age and race. But awareness, monitoring for potential signs and seeking prompt attention are always key ingredients in protecting your oral health!
